Rates vary a lot. Fine Laser Cut charges US$70 an hour to prepare files, with a US$15 minimum. LaserCutZ posts US$500 an hour and a US$750 minimum for setup and small runs.
A clean DXF or DWG at full size. A sketch works too, but the shop may charge to draw it.
